- Place the chicken breasts in the slow-cooker insert. Sprinkle evenly with the Italian seasoning mix, optional garlic powder, and black pepper.
- Whisk the condensed cream of chicken soup and low-sodium chicken broth together in a bowl until combined. Pour the mixture over the chicken and add the sliced mushrooms if using.
- Cover and cook on LOW for 4–6 hours or HIGH for 2–3 hours. Check the thickest portion with an instant-read thermometer; the chicken is ready when it reaches 165°F (74°C).
- Remove the chicken briefly from the slow cooker and shred or slice it. Stir the softened cream cheese into the hot sauce until smooth.
- Return the chicken to the slow cooker and mix it with the creamy sauce. Cover for another 10–15 minutes if needed so the cream cheese melts completely, then serve.
Notes
Soften the cream cheese before cooking and add it near the end for the smoothest sauce. Use chicken breasts of similar thickness, begin checking early, and avoid overcooking. Cream of mushroom soup may replace the cream of chicken soup.
