- Place the four chicken breasts in the slow cooker, arranging them in a single layer as much as possible.
- Place one slice of Swiss cheese and one slice of cooked ham on top of each chicken breast.
- Whisk the condensed cream of chicken soup and milk together in a small bowl until smooth.
- Pour the soup mixture over the chicken, ham, and cheese, coating the top of each portion.
- Cover and cook on LOW for 4 to 6 hours, checking the thickest piece with an instant-read thermometer.
- Serve the chicken with the creamy sauce from the slow cooker, alongside rice, mashed potatoes, or steamed vegetables.
Notes
For even cooking, pound thick chicken breasts to a uniform thickness or use thinner breasts or chicken cutlets. Chicken must reach 165°F (74°C) in the thickest part; avoid cooking well beyond that point because breast meat can become dry.
