  1. Place the four chicken breasts in the slow cooker, arranging them in a single layer as much as possible.
  2. Place one slice of Swiss cheese and one slice of cooked ham on top of each chicken breast.
  3. Whisk the condensed cream of chicken soup and milk together in a small bowl until smooth.
  4. Pour the soup mixture over the chicken, ham, and cheese, coating the top of each portion.
  5. Cover and cook on LOW for 4 to 6 hours, checking the thickest piece with an instant-read thermometer.
  6. Serve the chicken with the creamy sauce from the slow cooker, alongside rice, mashed potatoes, or steamed vegetables.

Notes

For even cooking, pound thick chicken breasts to a uniform thickness or use thinner breasts or chicken cutlets. Chicken must reach 165°F (74°C) in the thickest part; avoid cooking well beyond that point because breast meat can become dry.
